Certificate of visum et repertum or original autopsy certificate if needed from a legal and authorized Doctor
Original certificate from the police if the insured dies due to an accident
Original death certificate from the local Indonesian Consulate General, if the Insured dies abroad
Photocopy of the Policy Holder's passbook / Termaslahat
Other documents that BRI Life deems necessary relating to requests for payment of Insurance Benefits
Application form for withdrawing all Investment Value that has been filled in correctly and completely by the Policy Holder
