The original policy and changes (addendum) to the last policy
Death Claim Form that has been filled in correctly and completely by the Policy Holder / Insured
Photocopy of identity (KTP / SIM / Passport attached with KITAP / KITAS / KIM if foreign citizens) of the Insured, Policy Holder, Heir and those who apply
Photocopy of the family card of the Insured, Policyholder and Heir
If the heir is more than one person, then it must attach the original power of attorney from all authorized heirs to one of the heirs and have sufficient duty stamp
Original / legalized original death certificate from the competent authority
Certificate of visum et repertum or original autopsy certificate if needed from a legal and authorized Doctor
Original certificate from the police or doctor which provides information that the insured died due to an accident
Original death certificate from the local Indonesian Consulate General, if the Insured dies abroad
Photocopy of savings book including the Policyholder / Insured's account number
Other documents that BRI Life deems necessary relating to requests for payment of Insurance Benefits
Inpatient and outpatient claim forms that have been filled in correctly and completely and have been signed by the Insured
Original Certificate of Doctor claiming Inpatient and Outpatient which has been filled in by the examining Doctor
Original Certificate of a licensed Physiotherapist
Photocopy of identity (KTP / SIM / Passport attached with KITAP / KITAS / KIM if foreigners) of the Insured and the Policy Holder are still valid
Photocopy of all medical examination results (including but not limited to laboratory, radiology, medicine)
Photocopy of diagnostic results from specialist doctors and evidence of radiological examination results
Original all medical treatment receipts and bill details
Photocopy of passbook containing the account number of the Policyholder or the insured
Other documents deemed necessary by PT. BRI Life Insurance relating to the request for payment of insurance benefits
Death Benefit due to an accident in the amount of 100% of the Sum Insured
Reimbursement of Emergency Outpatient Expenses due to an accident during sports of a maximum of 5% (five percent) of the Sum Insured
Operational costs due to sports accidents at a maximum of 5% (five percent) of the Sum Insured)
Reimbursement of Physiotherapy and Recovery Costs due to sports accidents at a maximum of 5% (five percent) of the Sum Insured
Compensation for Fractures due to accidents during sports of a maximum of 5% (five percent) of the Sum Insured
Entry age 6 years to 64 years
Insurance period with options: 1 Day, 1 Week, 1 Month, 3 Months, 6 Months and 1 Year
One Time Premium Payment
Sum Insured is Rp.25,000,000,- and multiples thereof with a maximum Sum Assured for each insured of Rp.100,000,000,-

Suicide, attempted suicide or self-injury, if the act or event occurs within 24 (twenty four) months from the Insurance Start Date
Acts of crime or attempted crime by parties with an interest in the Police
Dehidrasi Berat atau Heat Stroke atau Cedera Kepala Berat atau Cedera Spinal Cord atau Strain atau Sprain atau Fisioterapi atau perawatan atau Patah Tulang (termasuk namun tidak terbatas pada kerapuhan tulang atau Osteoporosis atau penyakit tulang) yang disebabkan secara langsung ataupun tidak langsung oleh keadaan medis yang sudah sebelumnya baik yang diketahui ataupun tidak diketahui pengobatannya
Severe Dehydration or Heat Stroke or Severe Head Injury or Spinal Cord Injury or Strain or Sprain or Physiotherapy or treatment or Fractures occurring before the Policy Effective Date
Medical services or treatments performed and/or provided by the Insured or members of the Insured's family or by people who normally live in the same household as the Insured.
All costs are due to non-compliance and indiscipline of the insured towards medical care and treatment, and the personal benefit of a doctor or parties who have an interest
