Original certificate from the police, if the Participant dies due to an accident
Original death certificate from the local Indonesian Consulate General, if the Participant dies abroad
Photocopy of identity (KTP / SIM / Passport attaching KITAP / KITAS / KIM if foreigners) of the Participants
Photocopy of Participant's family card
